Question about tips in luxurious hotels such as Four Seasons?

How much tip are you supposed to give five star hotels such as the Four Seasons? Specifically, I would like to know how much tip to give to: 1.The person who helps you with the luggage 2.The person who takes you to your room 3.Daily maid service 4.If the breakfast is included, is the tip included as well? 5.What about the shuttle bus driver from the airport? By the way, I meant the hotels in Hawaii, US.

1. I starred you question because I am curious about the answers you may get. I think it will be different in the US than the overseas hotels where tips are often included in the food service, but not for the porters and maids...
2. Well I usually don't stay there, usually the Hilton or ritz, but it would be the same. When it's just me and my husband, I do about $20 each day but if I'm with my kids and husband, about $30.
